Personal Information Policy
I Want My Carpet Cleaned Inc. is committed to the safeguarding of personal information entrusted to us by our clients. We manage your personal information in accordance with Alberta's Personal Protection Act, and other applicable laws. This policy outlines the principles and practices we follow in protecting your personal information.
This policy applies to I Want My Carpet Cleaned Inc. The policy also applies to any person officially providing services on our behalf.
A copy of this policy can be provided to any client upon request, within due time.
WHAT IS PERSONAL INFORMATION:
Personal information means information about an identifiable individual. This includes an individuals' name, home address, phone number, age, sex, marital or family status, an identifying number, financial information etc.
WHAT PERSONAL INFORMATION MIGHT WE COLLECT:
We collect only the personal information that we may need for the purpose of providing services to our clients, including personal information needed to:
* Open and manage an account.
*Deliver requested products or services, or information.
*Enroll a client in a program.
*Send out association membership information.
*Provide warranties for products or services.
*Contact clients about appointments.
*Follow up on satisfaction of our products or services.
*Notify clients of our upcoming promotions or events.
*Administer a loyalty program.
*Grant credit or do a credit check.
*Meet regulatory requirements.
We normally collect information directly from our clients. We may collect personal information from other sources with your consent or as allowed by law. We may also collect personal information about our clients or others when, as an example: information is offered to a representative of this company with or without request by this company at a time such as a referral is offered in a situation where a person or company thinks we should contact another person or company in someway associated with the referrer. We inform our clients before or at the time of collecting personal information, and of the purposes for which we are collecting the information. However we do not provide notification when a client volunteers information for an obvious purpose. ( Such as when a client is producing a cheque or other form of payment that has identifying information printed on it, or address for the appointment being booked.)
AS PER SERVICE PROVIDERS OUTSIDE OF CANADA:
We currently have none at this time that would normally involve the transfer of personal information, when this becomes necessary we shall inform, as necessary those effected.
CONSENT:
We ask for consent to collect, use or disclose client personal information, except in specific circumstances where collection, use or disclosure is permitted by law. We may assume your consent when you volunteer information for an obvious reason. We ask for your express consent for some purposes and may not be able to provide certain services if you are unwilling to provide consent to the collection, use, or disclosure of certain personal information.
Where express consent is needed we will normally ask clients to provide their consent orally( by phone or in person), in writing(by signing a consent form, by checking a box on a form), or electronically(by clicking a button).
In cases that do not involve sensitive information, we may rely on an "opt out" consent. For example we may disclose information to other organizations that we feel may be of interest to you, unless you request that we do not disclose your information. You can do this by informing us orally, in person or by phone, or in writing. We do not offer to any person or organization any personal information, such as in the way of direct profiting by selling, renting, leasing or such.
A client may withdraw consent to the use and disclosure of personal information at any time, unless the personal information is necessary for us to fulfill legal obligations. We will respect your decision, but we may not be able to provide you with certain products and services if we do not have the information. We may collect, use or disclose client personal information without consent only as authorized by law. As an example we may not request your consent when the collection, use or disclosure is reasonable for an investigation, or legal proceeding, to collect debt owed to our organization, in an emergency that threatens life, health or safety, or when the personal information can be collected from a generally public source.
HOW DO WE DISCLOSE PERSONAL INFORMATION:
We use and disclose personal information only for the purpose for which the information is collected, except as authorized by law. For example we may use client personal information to deliver goods, or services. The law also allows us to use personal information for the use of collecting debt owed to our organization, should that be necessary. If we do wish to use your personal information for any new purpose we will ask for your consent.
HOW DO WE SAFEGUARD YOUR INFORMATION:
We make reasonable effort to ensure that your information is accurate and complete. We rely on our clients to inform us if there is a change to the information that may affect the relationship with our company. If you are aware of an error in our records about you, please let us know and we will correct it upon request wherever possible. In some cases we may need a written request.
We protect personal information in a manner appropriate for the sensitivity of the information. We make reasonable effort to prevent any loss, misuse, disclosure, or modification of personal information, as well as any unauthorized access.
We will notify the Office of the Information and Privacy Commissioner of Alberta, without delay, of a security breach affecting personal information if it creates a real risk of significant harm to individuals.
We retain client personal information as long as it is reasonable to fulfill the purposes for which the information was collected or for legal or for business purposes. We render client personal information non-identifying, or destroy records containing personal information once the information is no longer needed. We use appropriate security measures when destroying personal information, including but not limited to shredding paper records, and deleting electronic records.
ACCESS TO RECORDS CONTAINING PERSONAL INFORMATION:
Clients of I Want My Carpet Cleaned Inc. have a right to have access to their own personal information in a record that is in our custody or under our control, subject to some exceptions. For example: organizations are required under the Personal Information Act to refuse to provide access to information that would reveal information about another individual. Organizations are authorized under the Act to refuse information if disclosure would reveal confidential business information. Access may also be refused if the information is privileged or contained in mediation records. If we refuse in whole or in part any request we will provide the reasons for the refusal. In some cases where exceptions to access apply we may withhold that information and provide you with the remainder of the record.
You may make a request to access your personal information by writing to The President of I Want My Carpet Cleaned Inc. You must provide sufficient information in your request to allow us to identify the information you are seeking.You may also request information about our use of your personal information and any disclosure of that information to persons outside our organization, in the same manner, or in some cases by verbal communication. You may also request a correction of an error or omission of you personal information by reasonable means of communication.
We will respond to your request within forty five calendar days, unless an extension is granted. We may charge a reasonable fee to provide information, but not to make a correction. We will advise you of any fees, that may apply before beginning to process your request.
